Clarification and Explicit Consent Text on the Protection of Personal Data
In accordance with the Law on the Protection of Personal Data No. 6698 (“KVKK”);
I hereby give my explicit consent and approval for my personal data—including identity,
contact, health, and special categories of personal data within the scope of genetic/epigenetic evaluation
shared by me through this form—to be processed and stored by Prof. Dr. Halil Coşkun
in his capacity as the Data Controller, for the following purposes under Articles 5 and 6 of the KVKK:
- Evaluation of epigenetic test results,
- Creation of personalized health, nutrition, and longevity consultancy plans,
- Maintenance of medical records and execution of health services,
- Fulfillment of legal obligations arising from the relevant legislation.
I also consent to the sharing of this data, limited strictly to the purposes stated above and only when
necessary, with authorized laboratories, healthcare institutions, and persons/entities permitted by legislation.
I have been informed that my personal data will be kept securely for the periods prescribed by the
applicable legislation, and that I have the right to access, rectify, erase, destroy, object to,
and request information regarding my data within the scope of Article 11 of the KVKK.
In this context, I hereby acknowledge and declare that I have read and understood the clarification
text and that I give my explicit consent to the processing of my personal data for the specified
purposes of my own free will.
